A traditional org chart says who reports to whom — not who can actually do what. Skills were undocumented, growth paths were ad-hoc, and there was no objective signal for deciding who should own high-risk or sensitive work.
The gaps showed up operationally: assignments leaned on memory and proximity rather than verified capability, onboarding repeated itself inconsistently, and ambitious team members had no visible ladder to climb.
